The permanent haze
Condensation between the panes means the perimeter seal has failed and moisture is inside the airspace. It cannot be cleaned, dried out or repaired — the unit is done.
It also means the insulating performance you paid for is gone, which shows up on the energy bill before it shows up on the glass.
Replacing the unit, keeping the frame
In most cases the sealed unit comes out and a new one goes in, leaving the frame and the sash alone. That depends on the frame being sound and the unit being removable — some systems are glazed in a way that makes this harder than others.
The new unit gets matched on overall thickness, airspace, coating and tint so it performs and looks like its neighbours.

Straight answers
Can a fogged window be repaired instead of replaced?
No. There are services that drill and vent failed units, but that removes the sealed airspace that made it an insulating unit in the first place. The haze may clear; the performance does not come back.
Do I have to replace the whole window?
Usually not. If the frame is sound and the glazing method allows it, the sealed unit alone is replaced.
